For your first appointment as a new patient, you will need to provide information about your health and current medication. All patients under 18 need to have a parent or guardian with them.
Arriving early
All new patients need to complete our patient information form.
For your first appointment, please arrive at least 10 minutes early. This will give you time to complete the patient information form.
Items you need for your first appointment
Please bring the following with you:
- your health insurance card
- a list of all prescriptions and/or medications for the past 12 months
- X-rays, scans and relevant hospital reports.
Health insurance card
You will need to show our receptionist your health insurance card when you submit your patient form.
Current prescriptions and medication
Please bring a list of all your prescriptions and medications – and corresponding dosages – for the past 12 months. This will help with your diagnosis and treatment.
X-rays, CT scans, MRI scans and hospital reports
You should bring all results or reports from recent blood tests and recent hospitalisation. Also bring any X-rays, CT or MRI scans.
If you’ve had a previous diagnostic study, please ask your doctor to forward the films and results to our office.
Your doctor may not be able to send us your test results etc. before your appointment, so please bring them with you to your appointment.
Waiting times
We try our best to have short waiting times and to stay on schedule. However, emergencies and unforeseeable circumstances may cause delays and increase your waiting time.
We appreciate your understanding and patience if a delay occurs.
After your appointment
At the end of your consultation, we will let you know if you need a follow-up appointment. We will also tell you when your next visit needs to be.
Our receptionist will give you an appointment card with a reminder for your next appointment.
